Rick discovers in "Walking Dead" season 4 episode 5 titled "Internment" has discovered that everybody on death row is in danger...from themselves. Everybody there is either sick, or dying. Glenn and Hershel and Sasha do everything they can until Glenn collapses.

At one point, Dr. S shows Hershel a box with a gun and ammunition, saying that he was only being realistic.
Meanwhile, outside of the prison, the fences are collapsing due to the weight of zombies. Rick and Maggie use logs to keep them at bay, with RIck telling Maggie about what he did about Carol. Maggie understands.

And then, a gunshot. Rick tells Maggie to go and help despite the zombies pressing in eve further. With Maggie gone, Rick tries to run to Carl for help...and the zombies eventually succeed in collapsing the fence.
Back inside death row, almost everyone has become a walker. Maggie breaks in the viewing glass, saving Glenn, Hershel and Sasha. Outside, Rick and Carl are overrun by zombies.
At the end of the episode, Daryl, Michonne, Tyreese, and Bob return with the medicine to discover the chaos that had erupted.
The very last scene shows a distant figure watching the events from afar. It is the Governor.
